Brief
description: The Department of Surgical Oncology offers comprehensive care for
cancer patients through both inpatient and outpatient services. Since its
inception, this department has been working hard to promote excellence in
cancer surgery. It is one of the most important divisions of NICRH, providing
surgical inputs in the multidisciplinary management of cancers. Within the
department, there are five specialized units dedicated to organ-specific cancer
care services. These are –
i.
Upper
GI unit – This unit focuses on cancers in the upper digestive tract, including
the esophagus, stomach and small intestine. Radical gastrectomies with D2
lymphadenectomies are regularly performed with. Esophageal cancer surgeries
including two-field and three-field esophagectomies are also being done
successfully.
ii.
Lower
GI unit – All colorectal malignancies are successfully managed in this unit.
iii.
Breast
unit – We have a huge load of breast cancer patients at NICRH and the dedicated
breast unit of Surgical Oncology department excels in their surgical management.
Breast-conserving surgery (BCS) and Oncoplastic breast surgeries (OPBS) have
changed the approach to the management of these patients. We also routinely
perform hydrogel marker insertions, SLNB with dual tracers, wire-guided
breast-conserving procedures, and other latest surgical techniques.
iv.
Hepatobiliary-pancreatic
unit: This unit tackles challenging procedures, including Whipple's procedure
for pancreatic and periampullary cancers, extended gallbladder cancer surgery,
and biliary tract surgeries.
v.
Sarcoma
unit: This unit addresses various soft tissue sarcomas of various sites,
including retroperitoneal sarcomas. At present this unit also manages bone
sarcoma patients with regular use of endoprosthesis to facilitate limb salvage
procedures.
Furthermore, the
department conducts various diagnostic and therapeutic procedures as part of
OPD care, including core needle biopsies, incision-excision biopsies, endoscopy
with stenting, colonoscopy, endoscopic polypectomy, CT-guided core biopsy, ERCP
and stenting, PTBD etc.
In recent years, the
department has excelled at minimally invasive surgeries. Gastric, colorectal
and hepatobiliary-pancreatic surgeries are done laparoscopically on selected
patients with promising outcome.
b.
History
and establishment: Initially starting with outpatient service only, the
department formally started as an integral part of the 50-bed institute in
1991. Since then, the department has been graced and guided by various visionary
and charismatic surgeons, who will always be remembered with utmost respect by
all surgical oncologists of Bangladesh. The institute was upgraded to 150-bed
in 2009 and then to 300-bed in 2015, when the Surgical Oncology Department was
run as two units. The year 2021 marks a significant milepost in the history of
the department when for the first time organ based separate units were
established. We are proud to say that we are in a constant state of dynamism,
making every effort to keep pace with the advancements of cancer surgery all
over the world.
